Registration Information Please have a parent or guardian present with your child for registration. You do not need any paperwork other than the registration form and Jax Beach Form. Your coach or team mom will contact you for the additional items needed at a later date. View age & weight requirements for each division. Download the 2010 physical, registration form and Jax Beach form. Registrations are held at Wingate Park.
