Registration Information Please have a parent or guardian present with your child for registration. You do not need any paperwork other than the registration form and Jax Beach Form. Your coach or team mom will contact you for the additional items needed at a later date. View age & weight requirements for each division. Download the 2011 physical, registration form and Jax Beach form. Please read the following Papers: Code of Conduct, Parental Behavior Registrations are held at Wingate Park.
